Transcript Request

To request your transcript, please complete the Google form available through the Registrar's Office. It takes 24-46 hours for PMHS to send out your Transcript to the school you requested. It typically takes 1-2 weeks for it to show on the University/College portal. Your transcript will be sent directly to the institution to which you are applying.
